Credit Card Authorization Fax Form

Instructions

1. After subscribing on the web site, you will want to print this blank form from your browser (select Print from the File menu). This form will then need to be completed and faxed to WCG in order to authorize your payment by credit card.
   
   
2. Fax your completed authorization form to 972-481-1951. You may also provide your credit card information over the phone at 972-481-1950.
